Notice of Intent to Provide Home Instruction Form

The intent form should be submitted each year by August 15th for each child to the Superintendent of Portsmouth Public Schools, 801 Crawford Street, Portsmouth, VA  23704.  Please note, according to the Code of Virginia §22.1-254.1. C, "parents who choose to homeschool under the Home Instruction statute must file evidence of their children's academic progress annually by August 1st (only for children age 6 and older as of September 30)."

In order to admit any child that has been in Home School Education into Portsmouth Public Schools a letter should be written to the Superintendent of Schools requesting they be allowed to register in their zoned school.  The parent will receive a letter acknowledging the intent to re-enroll in public school.  The letter should be taken to the school along with the necessary documents; birth certificate, shot record, tests/grades, and proof of residence to assist in registration and proper grade placement.
